Department of Labor and Industry Announces Implementation of Electronic Filing Requirements

The Department of Labor and Industry (DOLI) has announced that as of August 31, 2020, non-EDI reports and documents must be filed electronically in the new online Work Comp Campus system.

The list of forms that are to be e-filed are as follows:
  • PPD Benefit – to supplement EDI
  • Benefits Addendum
  • Employee’s Dependent Information – to supplement EDI
  • NOID – Notice of Intention to Discontinue Benefits for reasons other than return to work
  • Submit Insurer Report – to report benefits paid for purposes of SCF Assessments
  • Annual Claim for Reimbursement
  • Initiate Dispute – Request for Certification of Dispute 
  • Dispute Response – Medical or Rehabilitation Response 
  • Motion to Intervene 
  • Election to Exclude – relatives of closely held corporation
  • Claim Access Authorization – to review DOLI file
  • Trading Partner Profile Registration – Apply to be an EDI trading partner
  • DSR – Disability Status Report
  • VRU Consultation Request – Vocational Rehabilitation Unit Consultation Request
  • RCR – Rehabilitation Consultation Report
  • R-2 – Rehabilitation Plan
  • R-3 – Rehabilitation Amendment
  • R-8 – Plan Closure Report
  • Plan Progress Report – Vocational Rehabilitation Plan
  • Retraining Plan
  • Rehab Provider Individual Registration 
  • Rehab Provider Registration – for rehab firms
  • Object to Penalty
  • Request for Guidance with an Unreported Injury
  • Notice of Appearance or Representation
